SICK NOTES

IS THIS YOUR FIRST SICK NOTE?

1. BEEN IN HOSPITAL?

If you have been in hospital and need to stay off work - the hospital is responsible for issuing your sick note, covering the full period until the date by which it is anticipated that you will be able to return to work. Please contact the hospital if they did not provide you with a sick note.

2.  BEEN ILL FOR LESS THAN 7 DAYS?

If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances involves a charge of £40.

Many employers have their own self-certification forms. If your employer doesn’t have its own form your can download a form by clicking on the link below. Please  print it, fill it in and hand it in to your employer. You do not need to see a Doctor.

3.  BEEN ILL FOR MORE THAN 7 DAYS?

Please download the form below, complete it and drop it into the surgery. The completed form will be ready to collect within five working days. We will send you a text when the form is ready for collection.

If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note. Please download the form below, complete it and drop it into the surgery. We will contact you to let you know when you can collect your Sick Note or we may contact you to arrange an appointment.
