New Patient Registration

amenbury.jpgIf you cannot complete a registration form or do not have proof of address or ID it will not affect your registration.  Please contact us on 01582 712021.

If you live within our practice area you are welcome to register with us.  Our reception staff will be happy to guide you through the procedure. Eligibility can quickly be confirmed from your address, so please provide proof of address by way of a recent utility bill and one form of photographic ID. 

You will need to complete a registration form (GMS1) and a health questionnaire- this will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be downloaded from the links below.

After registering with us it is often some time before your previous medical records arrive here.

Medical treatment is available from the date your registration is processed. Please contact reception for further information.

To register at the practice:

Please be aware that to register with the Practice you must live within the Practice's area (map in New Patients tab above).

Please bring the following along to the surgery you wish normally to be seen at (Harpenden or Wheathampstead):

1. Photographic identification

2. Proof of your address

3. Completed registration forms (see list of options below)  AND GMS1 Registration Form (downloaded from links below).
